Chronic Care Management (CCM) is a Medicare benefit that pays your practice every month to keep patients with two or more chronic conditions on track. GenieMD runs the entire program in your practice’s name — outreach, consent, care plans, monthly care management, documentation and billing. All you do is qualify the patient.
CMS created Chronic Care Management in 2015 as a Medicare Part B benefit. It pays physician practices to deliver at least 20 minutes a month of non-face-to-face care management to patients with two or more chronic conditions expected to last a year or more — a comprehensive care plan, medication management, 24/7 access to a care team, and coordination with the patient’s other providers.
It is reimbursed by traditional Medicare and covered by the large majority of Medicare Advantage plans, and it is billed monthly under your NPI. Because CCM requires only general supervision, your care team does not have to deliver it personally — which is exactly where GenieMD comes in.
Cardiology patients on Medicare rarely have just one diagnosis. A patient with heart disease and diabetes, or heart disease and obesity, meets the two-condition threshold. Any two of the conditions below, expected to last 12 months or more, qualifies a patient for the program.
CCM is one of the few programs that is good for the patient, good for the relationship, and good for the practice at the same time.
A licensed care manager checks in every month: medication reconciliation and adherence, home blood pressure and weight review, symptom screening, diet and activity coaching, and coordination with the PCP and pharmacy. Problems surface early — a heart-failure weight gain, a BP trend, a lapsed statin — and get routed to you before they become an ED visit or readmission.
Every call, text and care plan is delivered in your practice’s name. Patients experience a cardiology practice that looks after them all month, not just on appointment day. That is the kind of relationship patients stay in, talk about, and bring their spouse to.
Care managers are trained to recognize when a patient needs to be seen — new or worsening symptoms, an overdue follow-up, a missed echo, lipid panel or device check — and to book the appointment with your front desk. We also remind patients before scheduled visits, so fewer slots go empty.
CCM reimburses your practice per enrolled patient, per month, for as long as the patient stays in the program. It is billed under your NPI and paid to you. The downstream visits, imaging and procedures it generates are on top.
Based on the 2026 national average for CPT 99490 (first 20 minutes of clinical-staff CCM), roughly $66 per patient per month. Add-on time (99439), physician-delivered CCM (99491) and complex CCM (99487) reimburse more. Actual rates vary by locality and payer; this is a planning estimate, not a guarantee.
Most CCM programs fail because they land on the front desk and the MAs. Ours is designed so that your team’s involvement begins and ends with one step.
Confirm the patient has two or more chronic conditions — for example heart disease and diabetes, or heart disease and obesity. In practice that is a list from your EHR and your sign-off. That is the whole job.
A 15-minute conversation is enough to estimate how many of your patients qualify, what the program would mean for your practice, and how soon enrollment could begin.
