What is CCM, and how does it help me?
A dedicated care professional works with you and your doctor to manage your conditions between appointments. You get a care plan built around your own health goals, and someone who knows your history is always a phone call away.
- At least 20 minutes a month of check-ins outside your office visits
- A personal care plan you help create, reviewed as things change
- Coordination with your pharmacy, specialists, labs, and hospitals
- 24/7 access to a qualified professional for urgent health needs
Do I qualify?
You likely qualify if you have Medicare, or both Medicare and Medicaid, and live with two or more chronic conditions expected to last at least a year. Common examples include:
Only one practice can provide CCM for you in a given month, and you give consent once before you start. You can leave the program at any time by telling your provider.
What does it cost?
CCM is covered under Medicare Part B, so the usual cost sharing applies: your deductible and a copay or coinsurance each month the service is provided.
If you have supplemental or Medigap coverage, it typically covers that cost sharing.
If you have both Medicare and Medicaid, you most likely owe nothing at all.
